Applications

• Reside in the city of Pauls Valley, OK (have a Pauls Valley address)

• Children 0-18 years old can qualify

• School aged children must attend Pauls Valley schools or Whitebead school

• Homeschooled children will be considered on a case-by-case basis

• Government issued identification (US driver's license, passport, military ID, etc.)

• Proof of residence (utility statement, bank statement, pay stub, etc.)

• Proof of enrollment in Pauls Valley or Whitebead schools (for school age children)

Applications will not be processed until all the proper documentation has been received and the application has been completed in its entirety
